Exeter Hospital, Core Physicians, and ClearChoiceMD Urgent Care are excited to announce that they have formed a clinical affiliation that will improve quality and continuity of care for shared patients across the New Hampshire Seacoast.
The relationship currently includes ClearChoiceMD locations in Portsmouth and Epping and will expand to Plaistow in July when Core Physicians and Exeter Hospital open their new Plaistow Regional Healthcare facility that will house a ClearChoiceMD location in the same building as Core Physicians adult, pediatric and specialty practices and Exeter Hospital physical therapy.
The new clinical affiliation focuses on collaboration around coordinated healthcare services between Exeter Hospital, Core Physicians, and the ClearChoiceMD centers. With more people gravitating to an urgent care setting as a first stop when they need care, the new clinical affiliation will enhance the care coordination for patients if they are transferred to Exeter Hospital or Core Physicians and create a more integrated option for Core patients seeking access to urgent care services. For example, the three entities will be able to electronically communicate information that will ensure shared patients receive the best quality care possible.

The clinical affiliation includes a designated point of contact at ClearChoiceMD to ensure optimal communication and coordination of care for patients as they move between ClearChoiceMD, Exeter Hospital and Core Physicians primary care offices and specialists.

Hampers, MD, President and Chief Medical Officer of ClearChoiceMD Urgent Care said the clinical affiliation will allow the three entities to provide their communities with high quality, easily accessible, convenient, and affordable access to urgent care and other coordinated care services. “By partnering with Exeter Hospital and Core Physicians we will be able to exchange information and coordinate follow-up care in a timely manner. We will have access to Exeter’s full suite of advanced services including emergency care, advanced imaging, physical therapy, and multiple sub-specialties including orthopedics, neurology, and many others,” Hampers said. “This healthy partnership is truly a win-win for all.”
ClearChoiceMD Urgent Care has developed similar successful relationships with LRGHealthcare in the Lakes Region, and Catholic Medical Center in Manchester.
